FORM: Application for extension of time to appeal a WorkSafeBC prohibited action or reopening decision

Notices of appeal for WorkSafeBC prohibited action decisions or claim reopening decisions must be submitted to WCAT within 90 days after WorkSafeBC issues their decision.

It takes about 5 to 10 minutes to complete this form. Make sure you have the following information ready:
- The WorkSafeBC prohibited action decision number or claim number and date for the decision you are appealing
- For prohibited action decisions, the WorkSafeBC employer account number for the decision you are appealing
