Only use this form if you want to assign someone to act as your representative. You are not required to have one. This form authorizes the Workers’ Compensation Appeal Tribunal (WCAT) and WorkSafeBC (including the Review Division), to give confidential information about you or your business to your representative. It takes about 5 to 10 minutes to complete.

Information from WCAT for appeals visible on WCAT Online Services
If you have a professional representative and they have signed up for WCAT Online Services, or has previously provided email consent, they will receive appeal information on Compensation, Occupational health and safety, or Assessment matters by email. Other appeal and application types are not able available on WCAT Online Services at this time.
If your representative has not signed up for WCAT Online Services or consented to email previously, we will send information by mail.
