FORM: Notice of Participation – occupational health & safety interested parties | Workers' Compensation Appeal Tribunal (WCAT)

This form will be your formal notice of participation in an appeal. Submit this form to WCAT within 14 days of our invitation letter. If we do not receive a response from you within 14 days, the appeal will proceed without your participation.

It takes about 10 to 15 minutes to complete this form. Make sure you have the following information ready:

The WCAT appeal number for the appeal you are participating in
The WorkSafeBC employer account number associated with the appeal you are participating in

Questions marked (Required) must be filled in so that WCAT can update your participation in the appeal.
